JOB SUMMARYThe LVAD Coordinator RN is a licensed professional who uses the Company nursing professional practice model to coordinate patient care delivery by the health care team and coordinates evaluation, admission, discharge of assigned Left Ventricular Assist Device (LVAD) patients. Using the nursing process, the RN assesses the patient, identifies nursing diagnoses based on responses to health problems, implements an individualized plan of care, evaluates the patient's response and documents outcomes.
