Job DescriptionUnder general supervision, serves as an advanced practitioner, educator, consultant, collaborator, and change agent in the assessment, development, implementation, evaluation and maintenance of a system of care for a selected population of patients. Facilitates the development, implementation, and ongoing management of a model for professional practice that fosters the delivery of high quality, cost effective patient care as well as the professional development of staff.
