Job DescriptionUnder general supervision, serves as an advanced practitioner, educator, consultant, collaborator, and change agent in the assessment, development, implementation, evaluation and maintenance of a system of care for a selected population of patients. Facilitates the development, implementation, and ongoing management of a model for professional practice that fosters the delivery of high quality, cost effective patient care as well as the professional development of staff. Supports and participates actively in ongoing research and quality improvement activities.
