The role youâ™ll contribute: The Certified Clinical Transplant Social Worker is responsible for completing comprehensive pre-transplant psychosocial evaluations on pediatric, adolescent, adult and geriatric patients. The Social Worker documents in patient charts in a timely, efficient, and clinical manner. The Social Worker determines patientâ™s ability to manage his or her own care post-transplant/Ventricular Assistance Device (VAD) care, including transportation, support system/caregiver, financial status, housing, mental status, and functional status. The Social Worker works directly with the pharmacist and financial coordinator to ensure the patient has the financial resources to acquire needed medications post-transplant. The Social Worker actively participates in patient and family support and education through the monthly support group. Attends and participates in team meetings that are focused on patient care and treatment planning. Provides case management, crisis intervention, and counseling where clinically indicated to patients pre- and post-transplant. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Performs and generates timely documentation of comprehensive psychosocial assessment(s) which includes the following:
Assessing patientâ™s ability to understand and comprehend transplant and VAD implant process
Assessing patientâ™s motivation for transplant, living donation, and VAD placement
Evaluating patientâ™s mental and psychiatric status and referring to mental health treatment as clinically indicated
Assessing patientâ™s employment status, disability, and insurance coverage to determine areas of potential concern, making appropriate referrals to the transplant financial coordinator and/or pharmacy assistant
Assessing patientâ™s compliance and ability to adhere to medication regimen and treatment recommendations
Assessing patientâ™s social history and availability/stability of support system
Assessing patientâ™s substance use history and referring to treatment programs as necessary
Assessing patientâ™s functional status
Assessing patientâ™s caregiver and transportation plan
Assess patientâ™s ability to adhere to important lifestyle behaviors that could affect transplant, VAD or living donation outcomes
Collaborates with the multi-disciplinary team in establishing/documenting goals and treatment plan for the patient. Serves as a consultant and educator to the multi-disciplinary team regarding the psychosocial components of illness, disability, and hospitalization.
Participates with the initial patient orientation process and provides ongoing patient education on the transplant/living donation/VAD process.
Utilizes advanced competence to perform crisis intervention as clinically indicated. This includes follow-up with patient in clinic or the bedside to assess adjustment, caregiver/transportation concerns, mental status, and make referrals as necessary. Assists in patient advocacy, resource brokering, and information and referral services.
Provides patients with support and education surrounding end-of-life issues and advance directives.
Qualifications
The expertise and experiences you'll need to succeed:
Minimum qualifications:
Minimum of two years of experience in case management, healthcare social work and/or mental health counseling
Minimum of two years of transplant experience
Current active State of Florida license as a Licensed Clinical Social Worker
Certified Clinical Transplant Social Worker through the Society of Transplant Social Workers
