This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until January 29, 2021. The initial cut--off date for referral of eligible applications will be December 24, 2020. All other referrals will be sent on an as needed basis. The PACT (Patient Aligned Care Team) Licensed Practical Nurse will work to support the Primary Care Clinics.
