The Care Coordinator works collaboratively with the medical providers in implementing the plan of care for patients within Gastroenterology. The RN demonstrates leadership and mastery of practice in delivering efficient, comprehensive, and continuous patient-driven holistic care through active collaboration, communication and coordination of resources. RN serves patients with known liver disease including cirrhosis, hepatocellular carcinoma, autoimmune liver disease and other hepatic disease.
