Valley Children’s is seeking an experienced Nurse Navigator to join the team in it’s Mercy San Juan Perinatal Center located in Sacramento, CA. The Nurse Navigator is a specialty nurse who functions in the role of expert clinician, educator consultant and coordinator of care.
Collaborates with Providers, Nurses and the healthcare team to coordinate, monitor and evaluate the clinical care and services for patients and families
Provide clinical support to improve access to care, patient care, and outcomes
Serve as a resource and primary contact to assist the patient and family in navigating healthcare and the course of treatment
Use in-depth knowledge and skills, advanced judgment and clinical experience in the area of specialty to assists the patient, family, staff and physician in providing solutions for complex patient care issues and navigating the healthcare systems
Coordinates activities across the continuum of care, interdepartmentally as well as through multiple healthcare facilities and subspecialty providers
This position practices within the scope of the California Nursing Practices Act and in accordance with healthcare organizations policies and under the direction of the Chief Nursing Officer
Active California RN License
Bachelor's Degree in Nursing (BSN) required
Minimum 3 years’ experience in clinical nursing specialty
Extensive advanced knowledge of nursing principles, clinical practice, and healthcare systems is necessary.
Basic Life Support (BLS) is required within 30 days of hire or transfer into the position
