Desert Regional Medical Center is a 385 bed acute-care hospital classified as a stroke receiving center and level 2 trauma facility with an innovative , patient centered and evidence-based Rehabilitation Services Department. Our compassionate team provides a wide range of inpatient and outpatient services, including acute care rehabilitation,joint replacement & spinal surgery, neurosurgery, ICU, Telemetry, step-down care, skilled nursing, as well as outpatient therapy, hand and lymphedema clinics.
The Transfer Center Coordinator (TCC) will facilitate a smooth and patient-centered admission process for patients into Desert Regional Medical Center (DRMC) ensuring the appropriate level of care bed and financial screening. The TCN will use critical thinking skills to expedite admissions into DRMC while demonstrating DRMCï¿½s Mission, Vision, Values and Service Standards in all interactions with referring facilities, physicians,
DEPARTMENT SPECIFIC DUTIES: ï¿½ Coordinates and facilitates the admission of patients from other healthcare facilities to DRMC using transfer center policies and procedures ï¿½ Collects clinical information to verify patientï¿½s level of care matches type of bed ordered by physician ï¿½ Collaborates with DRMC on-call physicians and hospitalists to arrange accepting physician. Communicates with referring hospital to coordinate transportation arrangements and provide an assigned beds ï¿½ Place bed assignments for all patients in hospital computer system and RightBed, except for patients in Perinatal Services and NICU ï¿½ Manages inhouse transfers and ensures RightBed and hospital computer system are kept up to date ï¿½ Ensures safe and timely placement of patients based on admission status, physician ordered level of care, patientï¿½s clinical information and nursing unitsï¿½ admission and discharge criteria ï¿½ Maintains competence in all computer systems ï¿½ Maintains 90% accuracy with all information entered in computer system ï¿½ Maintains documentation of activities and recorded calls ï¿½ Participates in process improvement activities ï¿½ Supervises the Transfer Center Representatives
Required: Current California RN License Minimum of 3-5 years ER/ICU/tele clinical experience Demonstrated organizational and leadership skills Excellent communication skills ï¿½ phone etiquette, written and verbal Ability to manage multiple priorities Demonstrated ability to present self in positive and professional manner Current American Heart Association BLS Certification
