DESCRIPTION: Ensures that program/unit/activities operate as planned and services are delivered in accordance with relevant policies and procedures. Meets productivity assignments. Provides general supervision of assigned staff and program. Initiates disciplinary action and evaluation reviews. Assigns/directs staff as indicated. Collects data for monthly QA/PI/UR reports as delegated, ensures that all grant data is gathered and reported to the Director, and gathers customer satisfaction data as assigned. Provides comprehensive case management to 25+ homeless mentally ill consumers, completes Intakes if needed, outreaches new consumers at known homeless locations if needed, makes hospital and home visits if needed, facilitates support and educational groups if needed, develops service plans, provides supportive counseling, accompanies consumers to appointments as needed, drives consumers to appointments in agency vehicles as needed, makes referrals, links consumers to needed services, enters data into HMIS daily, monitors the HMIS computer program and runs reports/insures data quality, monitors cases that need to be terminated, provides training and development to staff, maintains the psychiatrist's schedule and facilitates clinic days, assigns daily assignments to staff, monitors staff productivity, conducts chart reviews, interfaces with various community agencies and leaders, and documents all work in consumers' medical records. Monitors all client treatment plans and intervenes when required. Ensures that charts are appropriately documented; ensures chart compliance with relevant Policies and Procedures, insurance and regulatory standards. Participates in crisis intervention when required. Follows up post incident.
REQUIREMENTS: Master's Degree in Social Work, Psychology, or Counseling with experience in the mental health and/or substance abuse field; OR a BA or BS may be substituted for the Master's Degree with a minimum of 10 years experience in behavioral health and working with the homeless population and/or at least 5 years of supervisory experience. Experience with the homeless preferable. Valid, unrestricted NJ Driver's License required.
