We are recruiting for a Medical Assistant - Bilingual (English-Spanish), Chronic Care Management to join Lacuna Health in Louisville, Kentucky.
The Medical Assistant will perform Chronic Care Management calls on a monthly basis to the Medicare population with two or more chronic diseases. The goal is to facilitate care for the patient in the ‘in-between’ provider's visits to prevent readmission to the hospital or repeat Emergency Department visits. The Medical Assistant listens for an increase in symptomology in order to facilitate visits to the provider's office.
Telephonic communications with patients in a fast- paced environment.
Maintaining transitions of care between the care continuum.
Coordinates the sharing of patient information outside the provider's office.
Helps to engage the patient and their caregivers in their care by building rapport.
Educates the patient on their specific chronic diseases specific to their symptomology.
Ability to document in multiple platforms including a CCM platflorm and multiple EMRs.
Ability to work on a telephony system.
Ability to comprehend and determine real-time escalation needs for busy practices.
Using excellent customer service, answers phones and responds appropriately to calls, inquiries, and escalations.
Adheres to and participates in Company's mandatory HIPAA privacy program/practices, Business Ethics and Compliance programs / practices, and Company policies and procedures.
Reviews and adheres to all Company policies and procedures and the Employee Handbook.
Participates in special projects and performs other duties as assigned.
Requires excellent interpersonal skills, written and verbal communication skills, flexibility, efficiency, prioritization and record keeping.
Requires independent decision making, self-motivation, the capacity to work under limited supervision, and the ability to add to the work environment through positive attitude and effective interpersonal interactions.
Must have or develop a working knowledge or understanding of, Medicare regulations, and interdisciplinary process.
Must be able to multi-task and have the ability to work in multiple electronic medical record systems.
Must be able to work efficiently with office equipment (e.g., computers, telephone headsets, right-fax and multiple computer screens).
Associate's Degree preferred
Certification from national Accrediting body of Medical Assistants preferred.
Minimum of four years of direct patient care experience in an internal medicine office.
Knowledge of chronic disease highly preferred.
