Job Summary:
Provides clinical leadership and program direction for home and palliative based clinical services within the Geisinger at Home (GaH) care model. Develops, implements and evaluates strategic initiatives focused on home-based interventions which include, but not limited to, complex case management, acute care treatment protocols, care coordination, quality improvement, advanced illness/palliative care and social determinants focused on the highest risk populations. Establishes an integrated approach to population management utilizing clinical and financial data in program design, implementation, and evaluation.
Establishes an integrated approach to staff development and education.
Maintains overall fiscal responsibility for the department and performs duties to support the goals and objectives of GaH. Responsible for the analysis, development and implementation related to programmatic growth and expansion of home-based clinical services, as well as palliative care services. Provides iterative and collaborative leadership to optimize the best outcomes for patients and defined populations.
Reports directly to Vice President, Operations Geisinger at Home (GaH) with a matrixed reporting relationship to Chief Medical Officer, GaH.

Education and or Experience:
BSN, RN with Bachelor’s degree required. Master's degree in Healthcare administration, nursing, or other business management preferred and agreement to pursue.
Minimum 5 years clinical or case management experience required.
Minimum 3 years management experienced in managed care, acute care, home health, or health care system management required. Multiple site management experience preferred.
Previous experience in staff management and program development required.
Working Conditions/Physical Demands:
Work is typically performed in an office environment but travel to patient homes required for staff supervision/evaluation. Involves occupational exposure to disease illness including, but not limited to, HBV and HIV viruses. Frequently stands, walks, sits climbs stairs, lift/carries up to five pounds, and drives an automobile.
